    目的 探讨乳腺癌术后化疗患者及其配偶二元应对的潜在类别,分析其影响因素,为临床制订针对性护理干预策略提供依据。方法 便利选取206对乳腺癌术后化疗患者及其配偶作为研究对象,采用一般资料调查表、中文版二元应对问卷、中文版感知压力量表、简易心理状况评定量表Kessler10进行调查。运用潜在剖面分析识别二元应对的潜在类别,采用无序多分类logistic回归分析探讨其影响因素。结果 乳腺癌术后化疗患者及配偶二元应对可分为4个潜在类别:协同抗压组(22.33%)、配偶主导组(40.29%)、患者相对主动组(16.99%)、全面匮乏组(20.39%)。患者职业状态、家庭人均月收入、患者及配偶的感知压力与心理困扰是二元应对潜在类别的影响因素(均P<0.05)。结论 乳腺癌术后化疗患者及配偶的二元应对存在明显的群体异质性,并具有不同的互动模式特征。护理人员需基于其二元应对特征及影响因素,实施以夫妻为整体、分类导向的精准干预策略,以提升夫妻双方的二元应对水平,改善其心身健康。

    Abstract:

    Objective To identify the latent classes of dyadic coping among breast cancer patients undergoing postoperative chemotherapy and their spouses, and to examine the influencing factors, so as to provide references for developing targeted nursing interventions. Methods A total of 206 dyads of breast cancer patients undergoing postoperative chemotherapy and their spouses were recruited through convenience sampling, then they were investigated by using a general information questionnaire, the Chinese version of Dyadic Coping Inventory, the Chinese Perceived Stress Scale, and the 10-item Kessler Psychological Distress Scale. Latent profile analysis was used to identify latent classes of dyadic coping, and multinomial logistic regression was conducted to explore the associated factors. Results Four latent classes of dyadic coping were identified among the participants:collaborative coping group (22.33%), spouse-dominated group (40.29%), patient more active group (16.99%), and overall deficient coping group (20.39%). Influencing factors included patients′ employment status, monthly household income, perceived stress and psychological distress in both patients and their spouses (all P<0.05). Conclusion Dyadic coping among breast cancer patients undergoing postoperative chemotherapy and their spouses exhibits significant group heterogeneity and distinct interaction pattern characteristics. Nurses should implement couple-based, classification-specific precise interventions based on the characteristics and influencing factors of the dyadic coping,so as to enhance dyadic coping and improve psychosocial health for both partners.
