Abstract:Objective To explore the effectiveness of a stratified management protocol based on continuous glucose monitoring in patients with type 2 mellitus diabetes (T2DM) , and to provide a reference for precision nursing of diabetic patients. Methods A total of 130 patients with T2DM who wore continuous glucose monitoring devices in the endocrinology outpatient clinic were divided into a control group and an observation group according to different wards, with 65 in each group.The control group received routine blood glucose monitoring management.In the observation group, patients were stratified into three layers:the intensive lifestyle intervention layer, the non-insulin antidiabetic agents treatment layer, and the insulin treatment layer.Stratified monitoring and health education were provided according to the layer.Glycemic outcomes, metabolic indicators, and the Summary of Diabetes Self-Care Activities (SDSCA) score were compared between the two groups before the intervention and at 3 months after the intervention. Results Finally, 61 patients in the control group and 60 in the observation group completed the study.After the intervention, apart from HbA1c, glycemic outcomes and metabolic indicators in the observation group were significantly better than those in the control group (all P<0.05).The SDSCA score in the observation group was significantly higher than that in the control group (P<0.05). Conclusion The stratified management protocol can effectively improve glycemic control and self-management ability of community-dwelling patients, thus to achieve precision diabetes care.
