    目的 探讨孕前超重/肥胖孕妇社会支持与身体活动障碍间的关系,并分析跌倒恐惧和恐动症对身体活动障碍的多重中介作用,为制订干预措施减轻其身体活动障碍提供参考。方法 2024年8-12月便利选取211名孕前超重/肥胖孕妇为调查对象,采用一般资料调查表、孕妇身体活动社会支持量表、孕期身体活动障碍量表、国际跌倒效能量表和恐动症量表进行调査,构建结构方程模型探索孕前超重/肥胖孕妇社会支持、身体活动障碍、跌倒恐惧和恐动症之间的因果关系路径,明确变量间的中介效应路径。结果 孕前超重/肥胖孕妇社会支持、跌倒恐惧、恐动症和身体活动障碍得分分别为(82.07±10.43)分、(37.06±5.69)分、(41.09±4.74)分、(91.84±16.45)分。中介效应分析表明,社会支持能直接预测身体活动障碍,直接效应占总效应的37.91%,跌倒恐惧、恐动症及两者的链式中介作用间接影响身体活动障碍,中介效应分别占总效应的25.39%、22.44%、14.26%。结论 孕前超重/肥胖孕妇社会支持对身体活动障碍存在直接影响;社会支持通过跌倒恐惧和恐动症的链式中介影响孕前超重/肥胖孕妇的身体活动障碍。早期精准评估社会支持、跌倒恐惧及恐动症情况,通过开展个体化健康教育和提升社会支持,引导孕前超重/肥胖孕妇进行跌倒恐惧的自我调控,减轻孕妇的恐动症,实施科学、精准的运动管理,以减轻其身体活动障碍。

    Abstract:

    Objective To explore the relationship between social support and physical activity barriers in pregnant women with pre-pregnancy overweight or obesity, and analyze the multiple mediating effects of fear of falling and kinesiophobia on physical activity barriers, so as to provide evidence for developing interventions to reduce their physical activity barriers. Methods A total of 211 pregnant women with pre-pregnancy overweight or obesity were recruited by convenience sampling from August to December 2024. The general information questionnaire, the Social Support Scale for Pregnant Women′s Physical Activity, the Barriers to Pregnancy Physical Activity Scale, the International Falls Efficacy Scale and the Kinesiophobia Scale were used for this survey. A structural equation model was constructed to explore the causal pathways among social support, physical activity barriers, fear of falling and kinesiophobia, and identify the mediating paths of the variables. Results The scores of social support, fear of falling, kinesiophobia and physical activity barriers of participants were (82.07±10.43), (37.06±5.69), (41.09±4.74) and (91.84±16.45) respectively. Mediating effect analysis revealed that social support could directly predict physical activity barriers, with the direct effect accounting for 37.91% of the total effect. Fear of falling, kinesiophobia, and their chain mediating effect indirectly affected physical activity barriers, accounting for 25.39%, 22.44% and 14.26% of the total effect respectively. Conclusion Social support exerts a direct impact on physical activity barriers in pregnant women with pre-pregnancy overweight or obesity. Additionally, social support influences physical activity barriers through the chain-mediated effects of fear of falling and kinesiophobia. Early and precise assessment of social support, fear of falling, and kinesiophobia is essential.Individualized health education shall be carried out and social support shall be enhanced to guide pregnant women with pre-pregnancy overweight or obesity to self-regulate their fear of falling, relieve their kinesiophobia, and implement scientific and precise exercise management, so as to reduce their physical activity barriers.
