Abstract:Objective To retrieve, appraise, synthesize, and summarize the best available evidence regarding discharge preparation services for heart transplant recipients, thereby providing an evidence-based foundation for standardized nursing practice in discharge preparation following heart transplantation. Methods A systematic search was conducted across domestic and international databases for evidence related to discharge preparation and transitional care for heart transplant recipients, including clinical decision support documents, clinical practice guidelines, best practice reports, evidence summaries, systematic reviews, expert consensus statements, and randomized controlled trials. The search period covered database inception through August 2025. Two researchers independently assessed the methodological quality of the included studies and extracted and synthesized the evidence according to thematic categories. Results A total of 15 studies were included, comprising three clinical decision support documents, one best practice report, three clinical practice guidelines, three systematic reviews, four expert consensus statements, and one cohort study. Ultimately, 40 pieces of best evidence were summarized across five domains:multidisciplinary team collaboration; assessment of physiological, psychological, and social adaptation at discharge; discharge planning; health education; and the establishment of a bidirectional follow-up model. Conclusion This study systematically synthesized the best available evidence regarding discharge preparation for heart transplant recipients and provides evidence-based support for continuity of care and discharge planning after transplantation. During evidence translation and clinical implementation, healthcare institutions should tailor interventions according to available resources and patient characteristics and optimize multidisciplinary collaboration models to enhance patients′ readiness for hospital discharge.
